Job Description

This is a TEMPORARY, 6-month assignment based out of Morrisville, NC! Must live within locality of Morrisville, North Carolina to be considered!

Length of assignment: 6 months with possibility for extension.

Detailed list of Key Responsibilities:

  • Conduct member outreach related to health promotion, wellness, chronic condition management, early childhood health and development, maternal health, substance use, tobacco prevention, and other population health initiatives.
  • Conduct outreach to members using telephonic, member portal, text, mail, and related modalities.
  • Assists in addressing members' barriers to care and refers to Care Manager as needed.
  • Collaborates with internal departments to support the success of health promotion programs.
  • The Specialist engages families through clear, compassionate communication; explains EPSDT benefits in understandable terms; assists with appointment coordination; identifies barriers to care; and connects members to Care Management or Member and Recipient Services as needed.
  • Documents all outreach interactions accurately to support compliance, quality reporting, and organizational performance goals
  • Conduct 50-70 outbound calls daily and ensure proper data entry and documentation

    • Competence in using technology applications to support member outreach and documentation such as electronic medical record/care management platform
    • Computer proficiency in Microsoft Office Suite including Excel, Word, Outlook and Teams
    • Ability to utilize computer equipment and web-based software to conduct work.
  • Other
    • Knowledge of health promotion, public health principles, social determinants of health, and implications in health promotion
    • Knowledge and understanding of basic medical terminology, disease process, physical and mental health conditions.
    • Ability to work independently with remote supervision
    • Understanding basic concepts such as social determinants of health and implications in health promotion.
    • Accountability and compliance ensuring all regulatory documentation requirements are met for member outreach
    • Professional conduct and high standard accountability.
    • Consistent, dependable attendance and adherence to schedule is critical for meeting outreach goals and ensuring timely follow-up with members.

Experience Needed:

  • Bachelor's degree, or similar, in health promotion, community health, health administration, public health or similar field
  • Preferably, Behavioral Health or Public Health background

Preferred Experience:

  • Bachelor's degree in health promotion, community health, health administration, public health or similar field
  • LCSW, Mental Health Advocate, Behavioral Health, Public Health background, license, or certifications
